It is a Village Ordinance that we charge $10.00 for one
certified copy of a birth certificate. Additional certified copies may
be obtained at the same time for the same person at $ 2.00 per copy.
The village only accepts cash, certified checks, or money orders.
If application is made by mail, only certified checks or money orders
will be accepted.
The only person(s) to obtain birth certificates must be
the natural Mother, Father, legal guardian (must show legal court
papers). The Father’s name must appear on the birth certificate in order
for the father to obtain a certified copy.
If a person wishes to get their own birth certificate,
they must be 18 years or older with a picture I.D. This also applies to
the mother, father or legal guardian; they must have a valid state I.D.
or a driver’s license. (picture I.D.)
To receive a certified copy of
a birth certificate through the mail do the following:

Name of the
person, complete date of birth

Mother’s
maiden name and married name

Father’s
name
-
If a person is writing for their own
certificate they need to be 18 or older and they must send a copy of
their driver's license or state I.D. (picture I.D.)

If the natural
mother or father is requesting a copy of their children's birth
certificate(s), they must send a copy of their driver’s licenses or state
I.D. (picture I.D.). If the mother is remarried, she should sign the
application just as she signed the certificate at the time of the birth
and then again with her current married name. If the father is
requesting the certificate, his name must be on the certificate and he
needs to sign the letter.

If
you are the legal guardian you must send a copy of the court papers
proving that you have legal guardianship of the child and also a copy of
your drivers license or state I.D. (picture I.D.)

A
self-addressed stamped envelope.
-
Money
order or cashiers check made payable to Village of Evergreen Park.
