Mission Statement
The Office of Citizens’ Services, also known as the OCS, acts as a liaison to Village Administration serving the residents of Evergreen Park. Our goal is to be responsive to the needs of the residents and organize programs based on those needs. OCS continues to work with the Evergreen Park Senior Citizens’ Council to advocate for seniors, to enact change and promote  intergenerational activities.  A bi-monthly newsletter is published, and activities are offered, which include daily exercise classes, weekly line-dancing, pinochle, movies, arts and crafts, large and regular print books, monthly theme lunches and medical equipment rental.

About OCS
OCS operates with three prt-time secretaries, two mini-bus drivers and two meals-on-wheels drivers, and office vounteers. The department strives to grow with the constnt change in technology and maintain curent information on Federal, State, and Local programs available.

Mini-Bus Schedule

The Office of Citizens’ Services mini-bus provides curb to curb transportation for Evergreen Park senior citizens and disabled adults.

 

The mini-bus operates Tuesday through Friday between the hours of 9:15 am to 3:30 pm.  No pick-ups are made between 11:50 am and 12:50 pm.

 

 For Bus Appointments

Call (708)422-8776 at least 24 hours ahead of time.  Appointments may be made as early as two weeks in advance. Time slots fill quickly.

  • We need your name, address and telephone number.

  • We need the time of your appointment, including the name, address and telephone number of your destination.

  • Inform us if you use a wheelchair.  If you do, you must have a companion ride with you.

  • Be ready 20 minutes before pick-up.

  • Bus fare is $.50 each way.

  • If you are a new rider we ask you to complete a registration form advising OCS of your personal demographics.  These records remain confidential.  Forms are available from the driver or the office.

Procedure For Return Trips

When finished with your appointment, call (708)422-8776 for return.  The driver will be notified by radio.  If you have found another way home, please notify the office.

 

Area Covered 

Medical & Dental:  Transportation is provided for medical and dental appointments within a 5-mile radius of Evergreen Park.

 

Banks:  Wednesday afternoon is designated as “Bank” day.  Appointments/pickups begin at 12:50 to banks in Evergreen Park only.  No bank appointments first Wednesday of each month.

 

Barber & Beauty Shops:  Evergreen Park establishments only.  Any time, space permitting.  (Medical appointments take precedence).

 

Center Activities:  Tuesday through Friday

 

Evergreen Plaza:  Wednesday afternoon - pickup begins at 12:50.

 

Grocery Shopping:  Thursday afternoon - Aldi’s.  Pickup begins at 1:00 p.m.  Lagen’s, any time space available.

 

Other Business in Evergreen:  Appointments as schedule permits.

 

Wheelchair/Special Equipment

Bus service begins at the curb.  If additional help is needed, the Office will arrange for paramedic assistance.  The mini-bus is equipped with a hydraulic lift to accommodate wheelchair clients.  Special needs clients must be accompanied by another person.  To allow for time and space, mention any special needs when making your appointment.

Provides service to Evergreen Park residents every Monday. Prior registration is requested.

 

 Senior Citizens Council

The Senor Citizen's Council was formed to act as an advisory group. To this day, the members are still very active. The Council meets on the third Thursday of the month at 11:30am. All residents are welcome!
